Job Description:

Archer Resourcing have an excellent opportunity for an adult social worker to join the team in South London.

The successful candidates will be joining a Hospital Discharge team that has well-established effective working partnerships with a variety of health professionals across the system. As a team, you will be working collaboratively to ensure that people are supported to be discharged from hospital in a safe and timely manner.

* Hours are flexible and based on shift work. Day and night shifts are available.

Responsibilities:

*Including but not limited to

• Complete safeguarding enquiries, mental capacity assessments and best interest’s decisions.
• Manage a complex case load and prioritise the competing demands and time pressures of ensuring timely discharges.
• Ensure that critical analysis and evidence-based practice is central to all decision making.
• Undertake strengths-based and person-centred assessments to meet individuals needs to be safely discharged from hospital
• Work collaboratively with professionals across health and social care to holistically meet individual’s needs.
• Support Students, ASYE’s and other colleagues with regular supervision and guidance.

Requirements:

• Enhanced DBS dated within the last 12 months - on the update service? You could be fast tracked through our recruitment process today
• Degree relevant and approved by SWE (Social Work England)
• Valid SWE registration
• Full U.K driving licence and access to own vehicle is advantageous
