Job description

Liquid Personnel is currently recruiting for an experienced and qualified Social Worker to join a busy Hospital Discharge Team based in Kingston.

Responsibilities include:
  • Ensuring compliance with conduct codes and legislation
  • Managing complex caseloads and assessing customer needs
  • Supervising staff and maintaining qualifications
  • Developing partnerships and promoting team practices
  • Ensuring safety, handling escalations, and staying updated on service developments
  • Conducting risk assessments and reviews
  • Communicating effectively and maintaining relationships
  • Sharing knowledge and contributing to team performance
  • Supporting development, acting on feedback, and ensuring high-quality service
  • Supporting managers, maintaining confidentiality, and working off-site
  • Challenging practices and adhering to standards
  • Undertaking additional duties as required
  • Specializing in case management, safeguarding, and occupational therapy
  • Conducting assessments, developing support plans, and ensuring timely interventions
  • Handling safeguarding concerns and recommending equipment and adaptations
  • Supporting rehousing assessments and progressing based on performance and standards
  • Demonstrating leadership and supervising staff to ensure quality and contribute to improvements

Qualifications and Experience:

Applicants must have experience in adult social care, conducting Mental Capacity Assessments, setting up care packages, and support planning. Essential requirements include Social Work England registration, eligibility to work in the UK, a full UK driving license, post-qualified experience, and a degree or equivalent in Social Work.
