Job Description:

Archer Resourcing has an excellent opportunity for an adult social worker to join the team in South London.

The successful candidates will join a Hospital Discharge team with well-established partnerships with various health professionals across the system. The team works collaboratively to ensure safe and timely hospital discharges.

Hours are flexible and based on shift work. Day and night shifts are available.

Responsibilities include:

• Completing safeguarding enquiries, mental capacity assessments, and best interests decisions.
• Managing a complex caseload and prioritizing demands to ensure timely discharges.
• Applying critical analysis and evidence-based practice in decision-making.
• Undertaking strengths-based and person-centred assessments for safe hospital discharges.
• Collaborating with health and social care professionals to meet individual needs holistically.
• Supporting students, ASYE’s, and colleagues with supervision and guidance.

Requirements include:

• Enhanced DBS check within the last 12 months (preferably on the update service).
• Degree approved by SWE (Social Work England).
• Valid SWE registration.
• Full UK driving licence and access to own vehicle are advantageous.
