NonStop’s client is a hospital based in South West London seeking experienced Qualified Social Workers for their Hospital discharge team. They focus on integrating adult social care and health services to create streamlined multidisciplinary services that prioritize individual needs and strengthen communities.

You will primarily facilitate safe and timely discharges for residents to their homes with the necessary support, working closely with health colleagues in a fast-paced environment. The team aims to promote residents’ independence and reduce hospital readmission risks.

Key Responsibilities:
  • Undertake Care Act assessments and Support Planning
  • Conduct safeguarding duties as an enquiry officer
  • Participate in team meetings and professional forums
  • Apply a strength-based and preventative approach to social work interventions
  • Perform Mental Capacity Assessments as required, in accordance with legislation
