Job summary

Focus CIC seeks an experienced Social Worker to join its adult teams, handling a varied caseload for clients aged 14+. You will conduct assessments under Care Act 2014 and Mental Capacity Act 2005, enabling people to live independently at home or in other settings.

You will collaborate with carers and professionals to provide high-quality, personalised care while adhering to safeguarding policies and mandatory reporting requirements.

Qualifications

• Experience of working in social care with vulnerable people.
• Excellent communication and report-writing skills.
• Ability to work collaboratively with carers, families and other professionals.

Responsibilities

• Provide high-quality, person-centred social care in partnership with individuals and carers.
• Manage a caseload, prioritise tasks, review needs and care plans, and monitor outcomes.
• Work within Mental Capacity Act and safeguarding policies to protect adults at risk.
• Prepare reports for panels, case conferences and court as required.
• Maintain accurate case records with confidentiality and timeliness.
• Adapt to agile working environments and support service users to maximise independence.
