Job summary

Bradford District Care NHS Foundation Trust is seeking an enthusiastic Band 6 Mental Health Liaison Practitioner to join the Acute Liaison Psychiatry Service (ALPS) on a 12-month fixed-term basis. You will work in a busy hospital setting providing timely mental health assessments and onward care planning.

The post emphasizes trauma-informed care, care for older adults, and pathways for intoxicated service users with opportunities for multidisciplinary collaboration and service development.

Qualifications

• Qualified health professional registered with the appropriate regulatory body (NMC/HCPC/S Wey) and post-qualification experience in mental health care.
• Experience of working in acute hospital and/or liaison psychiatry settings.
• Ability to work collaboratively across multidisciplinary teams and with external partners.

Responsibilities

• Provide a high standard of nursing care to service users within Bradford Acute Community Services.
• Carry out mental health risk assessments, formulations and short-term contingency planning for service users presenting in Accident & Emergency with mental health crisis.
• Deliver brief solution-focused interventions for service users, carers and families and signpost to relevant health and social care resources.
• Work within the Acute Liaison Psychiatry Service based at Bradford Hospital and contribute to partnership working with voluntary sector, local authority and blue light services.
