Job summary

Bradford District Care NHS Foundation Trust is recruiting a Mental Health Liaison Practitioner to support older adults in acute hospital and care home settings in Bradford and Airedale. You will liaise with professionals, assess, plan and deliver interventions, and signpost for further assessment as needed.

You will work with the Acute Liaison Psychiatry Service and other CMHTs, contributing to clinical governance, training and service improvement while upholding Trust policies.

Qualifications

• Registered Mental Health Nurse, Social Worker or Occupational Therapist required.
• Experience in older people’s mental health.
• Ability to liaise across hospital and care home settings.
• Knowledge of Mental Health Act (1983) and related policies.
• Strong communication and collaborative skills.

Responsibilities

• Accept and assess referrals for the liaison service and coordinate care.
• Provide expert input within acute hospital for older people with mental health needs.
• Work collaboratively with patients and carers to involve them in care planning.
• Contribute to resource planning, monitoring and standards of care.
• Apply the Mental Health Act in line with policies and legislation.
• Promote equality of opportunity and access for all service users and carers.
• Lead on governance, research, audit and best practice within the service.
• Develop communication networks across disciplines and agencies for continuity of care.
• Ensure care delivery aligns with professional guidance.
