Job summary

Wye Valley NHS Trust is hiring a Band 4 Therapy Assistant Practitioner for its acute therapy team, working in inpatient settings. You will contribute to physiotherapy and occupational therapy assessments and interventions across wards and home settings.

You will assess patients, plan discharges, and provide equipment to support independence, along with exercise-based rehabilitation to improve strength and function.

Qualifications

• Foundation degree/degree in Health and Social Care or Level 5 apprenticeship.
• Care Certificate or equivalent.
• Evidence of ongoing CPD and professional development.

Responsibilities

• Plan and deliver individual or team therapy tasks within scope of practice.
• Assess patients to determine safety for discharge and need for equipment.
• Provide exercise programmes and rehab to improve strength and function.
• Work autonomously and as part of a multidisciplinary team in hospital and home settings.
• Engage in service improvement activities and student education.
