Role Overview:

Looking for new challenges and opportunities to develop your skills? This role involves working in our award-winning primary care team to deliver pharmacotherapy services, including medicines reconciliation, monitoring high-risk medicines, and assessing patient medicines compliance. The successful candidate will work in general practices and centrally in our medicines reconciliation hub, with opportunities to be an independent prescriber or supported to become one.

Responsibilities:
  • Deliver clinical pharmacy services such as polypharmacy reviews and patient medicines compliance assessments.
  • Work with patients in their homes and care homes across the county.
  • Monitor service delivery and provide reports to stakeholders, including clinical leads.
  • Participate in practice-based research using quality improvement methodologies.
Requirements:
  • Experienced pharmacist with excellent leadership, communication, and interpersonal skills.
  • Enhanced PVG Scheme Membership.
