Are you an experienced Band 3/Band 4 Therapeutic Assistant Practitioner seeking a new and diverse challenge?
West Lothian continues to invest in delivering community assessment and rehabilitation closer to home through our Home First programme. This programme has seen significant investment in our community AHP teams and has recently secured funding to introduce a community single point of contact to enhance patient pathways.
We are looking for experienced Band 3 / Band 4 Therapy Assistant Practitioners to strengthen our growing team and deliver person-centered assessment and therapy across our pre-admission community pathways, discharge to assess function, virtual community hospital ward, and within our step-down Intermediate care facilities.
The successful applicant would join a very stable and dynamic multidisciplinary team with a passion for integrated working, putting patient outcomes and experience at the heart of their practice. This includes working closely with inpatient colleagues to identify and facilitate early supported discharge and provide community rehabilitation.
