Job description

This is an exciting opportunity to join an established Community Nursing workforce and be part of the Stewartry Home team.

This band 5 post covers Gardenhill, Dalbeattie and Kirkcudbright GP surgeries in the heart of Stewartry, you must be NMC registered and driving licence is essential. If you have a disability which precludes you from holding a driving licence, consideration will be given if alternative transport can be arranged.

You will be expected to support patients who are registered with all three practices. The hours are full time however, part time hours may be considered.

The post holder will be expected to provide high quality effective care within the community and work in partnership with patients, professional health care providers, care providers and other agencies. Clinical work includes wound management, catheter care and supporting patients at end of life.

The aims of the service to focus on prevention, early intervention and support self management to enable people to live longer, healthier lives at home. Good communication skills and the ability to work autonomously within the team is essential for this role.

The Community Nursing Service operates and open access policy available 365 days a year, 24hrs per day. The service does not operate on a waiting list; care needs are prioritised, and the team respond to planned and unplanned care. Unscheduled visits and those who require urgent intervention should be responded to timeously. The post holder will be delivering care to patients as delegated by the Community Charge Nurse. The caseload population is derived from the GP practices aligned to the service.
