A governmental health service in Saint-Cloud, France seeks a Licensed Clinical Social Worker to conduct assessments, coordinate care, and provide advocacy for Veterans. The ideal candidate will possess a Master's degree in Social Work and have experience in crisis intervention and mental health practices. Responsibilities include maintaining clinical records, conducting psychosocial assessments, and participating in performance improvement processes. This position is not authorized for relocation assistance or sign-on bonuses.

This position is eligible for the Education Debt Reduction Program (EDRP), a student loan payment reimbursement program. You must meet specific individual eligibility requirements in accordance with VHA policy and submit your EDRP application within four months of appointment. Program approval, award amount (up to $200,000) and eligibility period (one to five years) are determined by the VHA Education Loan Repayment Services program office after complete review of the EDRP application.

Duties
Clinical Functions
  • Conducting behavioral health and mental health biopsychosocial assessments.
  • Making sound professional judgments regarding clinical impressions, treatment approaches, and treatment planning.
  • Coordinating with internal and external professionals to ensure appropriate transitions of care and appropriate/necessary treatment is provided/offered to Veterans.
  • Establishing and maintaining knowledge of VA and Veteran's benefits and services, community resources, and processes for appropriate referrals to community and other governmental programs or agencies.
  • Providing advocacy to overcome potential barriers to access care/services in the best interests of the Veteran.
  • Providing goals of care discussions, as well as education and assistance in the completion of Advance Directives, Organ, Tissue, and Eye Donation release of information forms, and Guardianship documentation.
  • Assessing for signs and symptoms of abuse, neglect and exploitation.
  • Conducting practice in accordance with the VA and discipline‑specific ethical requirements and practices within the limits of current licensure or certification.
  • Completing on‑call duties as assigned and per policy, which may include transportation of a Veteran.
Administrative Functions
  • Possesses and demonstrates ability to communicate effectively, both orally and in writing with people from varied backgrounds.
  • Possesses skills in using computer systems and the internet for Veterans who choose this modality to communicate.
  • Serves on supervisor‑approved committees, work groups, and task forces at the local facility, VISN or national level, collaborating in performance improvement processes and complying with performance measures as required by the VA for specific population/program.
  • Attends and participates in all required administrative and clinical meetings.
  • Maintains accurate, timely, and complete clinical records in accordance with professional practice and VA administration guidelines.
  • Maintains data and statistical compilations to comply with accreditation bodies such as CARF and Joint Commission, and VA health care systems policies and procedures.
  • Performs other duties as assigned and can, at times, assist other incumbents or perform special assignments to meet the needs of the healthcare system.
  • May be assigned to other areas of need in the health care system, as determined necessary by the direct supervisor and/or the Integrated Care Community (ICC) Director in consultation with the Social Work Executive.
Role‑Specific Tasks
  • The HCHV Case Manager independently performs case‐finding functions and outreach, identifying and screening patients for psychosocial needs.
  • Provides crisis intervention services, seeks to address the cause as well as the presenting complaint, coordinates family conferences and serves as liaison to family members. Social worker will make rapid assessments and develop crisis management plans.
  • Provides wellness/prevention education and facilitates Patient/family support groups as appropriate.
  • Supervises the practice of social work students or a less experienced social worker, as appropriate.
  • Acts as liaison between VA and community resources, including marketing and public‑relations duties, as needed and approved per supervisor and policy.
  • Operates a government vehicle as an essential function of the job and could be required to transport patients in a government vehicle, even one time per year. Must maintain a valid state driver’s license and complete any annual physical or education requirements to maintain driving requirements of the position.
  • Coordinates, implements, and reports on community development activities on behalf of Veterans experiencing homelessness to the program coordinator, facility, VISN, and National Homeless Leadership, as directed by the MH Special Programs Manager. Engages with data collection, administrative record keeping, and monitoring patient or program clinical indicators, thresholds and performance measurements.
Work Schedule

Monday‑Friday 8:00 a.m. – 4:30 p.m. Evenings and weekends may be required.
