Empath Health is currently seeking a Bilingual Registered Nurse Case Manager (Spanish/English) to join our team.

This position is Monday - Friday, daytime hours. The coverage area includes North Hillsborough County/ South Tampa. This individual will see hospice patients in their homes, ALFs, and LTC facilities.

Empath Health is currently seeking an RN Case Manager to join our team.

The RN Case Manager uses nursing processes and case management strategies to coordinate care and provide nursing services for hospice patients and their families in private homes, assisted living facilities, and long-term care communities.

Primary tasks, duties, and responsibilities:

1. Under the direction of a licensed physician, manages discomfort and provides symptom relief using specialized nursing skills related to palliative and end-of-life care.
2. Provides initial and ongoing assessment of the impact of the terminal diagnosis on the patient's physical, functional, psychosocial, and environmental needs and activities of daily living.
3. In collaboration with the other members of the Interdisciplinary Group (IDG), develops, implements, and updates the individualized plan of care; initiates appropriate preventive and rehabilitative procedures; and initiates referrals to other services.
4. Educates patients/residents, families, and caregivers regarding disease process, decline, palliative interventions, caregiving, dying process, and safety practices.

Educational/Professional Requirements:

• Graduate of an approved nursing program and current unrestricted Florida RN license.
• Valid and current CPR certification required.
• Minimum one year experience in an acute-care hospital environment providing direct patient care; previous hospice RN experience preferred.
• CHPN certification encouraged within three years of employment.
• BSN or MSN preferred.
