RN Case Manager Float - Hillsborough County - Bilingual

Empath Health is currently seeking a Bilingual Registered Nurse (Spanish/English) to join our team.

Monday - Friday, 8am - 5pm. Primarily covers Hillsborough County with potential to cover Pinellas County. Will see patients in homes, ALFs, and LTC facilities.

The RN Case Manager:

1. Identifies and coordinates patient and family needs, providing preventative and supportive care to terminally ill patients and their families.
2. Utilizes assessment, intervention, and teaching skills to maximize patient comfort and enhance quality of life for the patient and family.

Position Requirements

1. Education and/or Experience: Associate's degree (A.A.) or equivalent from a two-year college or technical school; at least one year of related experience and/or training; or an equivalent combination of education and experience.
2. Licensure:

• Valid Florida license as a Registered Nurse
• Valid Florida State Driver's License

Bilingual - fluent in English and Spanish
