Marin Post Acute is seeking a compassionate, motivated Occupational Therapist (OT) to join our rehabilitation team. We provide post-acute and skilled nursing care focused on helping residents regain independence and improve daily function in a supportive, collaborative environment.

Applicants may be experienced or new graduates. We offer competitive pay and flexible scheduling options (Full-Time, Part-Time, PRN) with benefits including medical, dental, vision, life insurance, PTO, and a 401(k).

Qualifications

  • Active and unrestricted California OT license required.
  • Graduate of an accredited Occupational Therapy program.
  • Strong clinical assessment and treatment skills.
  • Excellent communication, documentation, and interpersonal skills.
  • Ability to work independently and collaboratively within a multidisciplinary team.

Responsabilités

  • Perform occupational therapy evaluations and assessments.
  • Develop individualized treatment plans based on resident goals and clinical needs.
  • Implement therapeutic interventions to improve mobility, strength, coordination, cognition, and ADLs.
  • Assist residents in regaining independence and maximizing functional abilities.
  • Educate residents, families, and caregivers on treatment plans and adaptive techniques.
  • Recommend and train residents on adaptive equipment and environmental modifications.
  • Collaborate with physicians, nursing staff, therapy team members, and interdisciplinary care teams.
  • Maintain timely and accurate clinical documentation in compliance with facility requirements.
  • Monitor resident progress and modify treatment plans as appropriate.
  • Participate in care conferences and discharge planning.

Occupational Therapist Responsibilities
  • Perform occupational therapy evaluations and assessments
  • Develop individualized treatment plans based on resident goals and clinical needs

  • Assist residents in regaining independence and maximizing functional abilities
  • Educate residents, families, and caregivers on treatment plans and adaptive techniques

  • Collaborate with physicians, nursing staff, therapy team members, and interdisciplinary care teams

  • Monitor resident progress and modify treatment plans as appropriate

Qualifications
Required
  • Active and unrestricted California Occupational Therapist (OT) License
  • Graduate of an accredited Occupational Therapy program
  • Strong clinical assessment and treatment skills
  • Excellent communication, documentation, and interpersonal skills
  • Ability to work independently and collaboratively within a multidisciplinary team
Preferred
  • Experience in Skilled Nursing Facilities (SNF), Post-Acute Care, Rehabilitation, or Long-Term Care
  • Knowledge of Medicare, PDPM, and rehabilitation documentation practices
