Job Title: Program Manager, Center of Excellence for Latino Health

Department: Community Health Department Clara Mass Medical Center

Reports To: Assistant Vice President of Community Health and Outreach

Job Summary:

Under the direction of the Community Health Department, the Program Manager for the Center of Excellence for Latino Health is responsible for designing, implementing, and evaluating culturally responsive health initiatives that serve the Latino community. This position plays a key role in improving access to care, addressing social determinants of health, and fostering partnerships that promote equity and wellness throughout the Clara Maass Medical Center service area.

Key Responsibilities:

  • Provide comprehensive patient navigation services across CMMC to assist patients in understanding and accessing healthcare services.
  • Conduct regular rounds on inpatients associated with the Center of Excellence for Latino Health to assess cultural, social, and medical support needs.
  • Support patients in navigating medical appointments, and overcoming logistical or financial barriers.
  • Develop programs that respond to community health needs assessments and promote culturally appropriate care delivery.
  • Represent the Center of Excellence for Latino Health and Community Health at meetings, events, and collaborative initiatives throughout the CMMC catchment area.
  • Serve as a bilingual (Spanish/English) liaison between Clara Maass Medical Center and the Latino community to enhance trust, outreach, and communication.
  • Coordinate and manage health initiatives for the Latino community to improve wellness and access to healthcare through activities such as educational sessions, screenings, workshops, and health fairs.
  • Establish and maintain partnerships with community leaders, faith-based organizations, local agencies, and stakeholders to strengthen collaboration and community trust.
  • Maintain program documentation, track data, and assist with reporting for internal use, grant support, and continuous improvement.
  • Develop culturally appropriate materials and messaging in both English and Spanish.
  • Contribute to other community health functions and projects as needed, in alignment with departmental objectives and evolving community needs.

Required Qualifications:

  • Bachelor s Degree in Public Health, or a related field preferred.
  • Fluency in Spanish and English is required.
  • Prior experience in community outreach, patient navigation, or public health program coordination (especially within Latino communities) is preferred.
  • Experience developing and facilitating community-based health programs.

Skills & Abilities:

  • Strong interpersonal, communication, and organizational skills.
  • Ability to take initiative, work independently, and manage multiple priorities.
  • Proficiency in data collection, reporting, and computer applications (e.g., Microsoft Office).
  • Excellent problem-solving skills and attention to detail.

Must be able to work a flexible schedule including evenings and weekends, and attend key community events and outreach programs.
