Location: Belleville and Quinte West Community Health Centre (BQWCHC)

Type: Temporary Full Time (1 Year)

Salary: $29.11 - $35.40 hourly

Job Summary

The Thrive Case Manager provides supportive counselling, time-limited case management, and advocacy for/with pregnant and/or mothering women and their children (ages 0-6), who report past or present substance use. The role involves developing and implementing education, health promotion, and skills-based learning programs, using a solution-focused, strengths-based, and harm reduction approach to support clients. The Case Manager works independently and collaboratively with community providers to address clients' health needs, including substance use issues, psychosocial and practical needs.

The position involves identifying and linking support services to women individually and within their family units. The Thrive Case Manager may accompany clients in interactions with clinical care teams, justice system personnel, child welfare agencies, daycares, and other services. The role also includes participating in service capacity-building across Hastings and Prince Edward Counties.

Key Responsibilities
  • Triage referrals, conduct client assessments, and program intakes.
  • Provide supportive counseling, practical support, advocacy, and time-limited case management in areas such as parenting, pregnancy, child development, family health, and managing health issues like addiction.
  • Assess psychosocial needs and develop individualized care plans, coordinating services accordingly.
  • Support clients in high-risk situations, including developing Safety Plans to mitigate crises.
  • Assist clients in accessing community services/resources and advocate on their behalf.
  • Engage with clients via phone, in-office, community visits, and home visits as needed.
  • Participate in the development, promotion, and evaluation of educational and health promotion programs for women who use substances and are pregnant/parenting.
  • Conduct and participate in public education sessions to raise awareness about substance issues among pregnant and parenting women.
  • Collaborate in regional Community Of Practice and team-building activities, utilizing trauma-informed practices.
Qualifications
  • Undergraduate degree in a relevant field or appropriate combination of education and experience.
  • Experience in client advocacy and navigation within community settings.
  • Experience working with individuals facing barriers such as substance use, mental health issues, complex social situations, and marginalization.
  • Ability to maintain professional boundaries, manage risks, and provide outreach services.
  • Knowledge of social determinants of health and their impact on access and wellbeing.
  • Knowledge of child welfare, local health, social service, and justice systems is an asset.
  • Strong critical thinking, problem-solving, and client advocacy skills; ability to work independently.
  • Excellent interpersonal and conflict resolution skills.
  • Basic proficiency in MS Office; electronic medical records experience is an asset.
  • Valid driver’s license and reliable vehicle.
