Responsibilities:

  • Provide direct patient care with a harm reduction approach, focusing on HIV and Hepatitis C treatment.
  • Conduct health assessments, diagnoses, and treatment planning.
  • Prescribe medications and monitor patient progress.
  • Educate and counsel patients on preventive care and chronic disease management.
  • Collaborate with physicians and healthcare teams to ensure comprehensive patient care.
  • Maintain compliance with CNO standards, medical directives, and privacy regulations.
  • Document patient interactions in electronic medical records (EMR).
  • Engage with community partners to enhance patient support services.

Qualifications:

  • Registered Nurse Practitioner (NP-Adult or NP-Primary Health Care) with valid CNO registration.
  • Masters degree in Nursing (MN-NP) or Master of Science in Nursing (MScN).
  • Experience in primary care, harm reduction, HIV, Hepatitis C, or community health (preferred).
  • Strong clinical assessment, diagnostic, and prescribing skills.
  • Excellent communication and patient education abilities.
  • Ability to work independently and collaboratively in a multidisciplinary team.
  • Proficiency in electronic medical records (EMR) systems.
  • N95 Mask Fit Test Certification & up-to-date immunizations (MMRV, TB test, or chest X-ray).
