Jefferson County Community Supports Program seeks a qualified professional to provide psychotherapy, clinical assessments, and case management for adults, teens, and children with severe mental illness. The role emphasizes coaching, treatment planning, and leveraging community resources to support recovery and independent living.

The position includes responsibilities for symptom monitoring, crisis intervention, and facilitating rehabilitation services and skills development.

Qualifications

• Master’s Degree in Social Work or Counseling with related experience.
• Licensed Social Worker or Licensed Professional Counselor preferred.
• Experience in community-based mental health services is desirable.

Responsabilités

• Provides psychotherapy, clinical assessment, and treatment planning for adults, adolescents, and children.
• Conducts case management using community resources and crisis intervention when needed.
• Monitors client symptoms and develops measurable treatment goals.
• Offers rehabilitation services and ongoing assessments of social, life, and parenting skills.
