Advocate Health is seeking a part-time Social Worker to perform psychosocial assessments and support patients and families during post-hospital care planning.

You will collaborate with the Clinical Care Management team to determine appropriate care levels, coordinate referrals, and connect patients with community resources.

Qualifications include a Masters in Social Work, LCSW preferred, and experience in hospital settings; flexible schedule with weekday hours.

Qualifications

• Masters in Social Work required; LCSW preferred.
• Applicable state certification preferred.
• 1 year professional hospital or health setting experience preferred.
• Expertise with data management tools.

Responsabilités

• Completes psychosocial assessments for post-hospital care.
• Assists patients coping with stress from hospitalization.
• Works with Care Management to determine care level.
• Serves as resource on emotional and social aspects of illness.
• Educates patients on community resources; refers to agencies.
• Reports abuse/neglect and liaises with social services.
• Responds to after-hours consults; notes on office setting and mobility.
