RHA Health Services seeks a Care Manager to cover Pender County and surrounding areas in a hybrid community-based role. The candidate will coordinate care across physical and behavioral health, I/DD, and community resources, traveling within the county as needed.

The position requires a master’s or bachelor’s in a human service field and relevant care management experience, with strong collaboration across teams and adherence to ethics and compliance.

Qualifications

• Education: Master’s or Bachelor’s in a human service field with required experience.
• Experience: LTSS/HCBS coordination, care delivery monitoring, and care management experience.
• Licenses/requirements: Valid driver’s license; clinical license preferred.

Responsabilités

• Deliver and coordinate comprehensive care management across sites and home/community settings.
• Lead care planning with persons supported and care teams; update plans annually or as needed.
• Collaborate跨-functional with Integrated Care Team, providers, CIN, and community partners.
• Maintain person-centered care focusing on health, wellness, safety, and self-management.
• Ensure data sharing and storage aligns with policies and supports care outcomes.
• Uphold policies for safety, quality, and compliance; monitor performance and outcomes.
• Communicate regularly with persons supported, families, and teams; build external relationships.
• Support ethics, confidentiality, and incident investigations as per policy.
