Job Overview:

The Mental Health Specialist will assist in navigating patients in need of mental health, substance abuse, and primary care services, ensuring that social determinants of health issues are addressed. The specialist acts as a member of the interdisciplinary team across various access points including the Emergency Department, PESS/Crisis units, inpatient units, and community providers. They will initiate post-discharge follow-up for mental health services and support adherence to aftercare appointments.

Qualifications:

Required:

  • High School diploma or equivalent.
  • Reliable transportation.
  • Skills in email communication, Microsoft Word, and other software systems.

Preferred:

  • BA in Social Work preferred.
  • Experience as a medical office assistant, medical assistant, or in a medical access/call center is a plus.
  • Bilingual skills are a plus.

Scheduling Requirements:

  • Full Time
  • Day Shift

Essential Functions:

  • Assist patients in overcoming barriers to appointments, such as arranging transportation or providing transportation via company vehicle.
  • Conduct outreach to patients and community partners as needed.
  • Maintain effective communication within the team.
  • Complete assessments and documentation to support NJ-QIP program requirements, ensuring timely data collection.
  • Build relationships with key stakeholders.
  • Gather patient information for documentation.
  • Initiate post-discharge follow-up and support adherence to appointments, communicating discharge plans with the team.
  • Meet patients in various settings to engage and guide them to proper follow-up care.
  • Collaborate with the multidisciplinary team on relapse prevention plans.
  • Provide motivational interviewing and timely feedback on patient progress, documented in the patient's record and communicated verbally.
