MHC Healthcare is seeking a Behavioral Health Care Coordinator to join the Behavioral Health team at the Marana Main Health Center, located in the heart of Marana, AZ.

The Behavioral Health Care Coordinator provides direct client services for coordinating care for behavioral health clients, ensuring smooth transitions between hospital and community-based services, and serving as a central point of contact for information and coordination on a rotating basis. This position plays a critical role in facilitating communication among hospitals, internal teams, and external stakeholders to promote continuity of care, client engagement and timely service delivery.

MHC Healthcare is a Federally Qualified Community Health Center (FQHC), with 13 sites in Tucson and Pima County. Our mission is to improve our Community by providing exceptional, whole-person healthcare.

The following qualifications are required:

  • Bachelor's degree in social services or related field.
  • 2 years’ experience in Behavioral Health and/or Case Management
  • Strong communication, organizational, and problem-solving skills.
  • Basic Life Support (BLS) for Healthcare Professionals certification (may be completed with MHC– must be completed within 1 month of start date)
  • First Aid certification (may be completed with MHC upon hire)
  • Level 1 Fingerprint Clearance Card through the Arizona Department of Public Safety (must be verified as “valid” prior to start date)
  • Current Arizona driver’s license with clean driving record and proof of current vehicle insurance (39-month MVR will be run by MHC)

The following qualifications are preferred:

  • One year experience with the State Regional Behavioral Health Authority (RBHA) system, AHCCCS Complete Care (ACC) health plans.
  • Experience working with diverse populations, including individuals with serious mental illness, substance use disorders, or co-occurring conditions.
  • Crisis Prevention Intervention (CPI) trained

Equivalent combination of education and experience may be considered if applicable and must be directly related to the functions and body of knowledge required to successfully perform the job.

Staff working strictly from home or remote may not be required to meet all of the noted requirements. Staff in hybrid roles (working partially on site in MHC facilities) are required to meet all stated requirements. Some requirements may be based on assigned site.

This position has the following supervisory responsibility:

Does not direct or supervise others.

The ideal candidate will also possess the following knowledge, skills, and abilities:

  • Ability to coordinate services among internal providers, primary care, specialty care, social services, and community resources.
  • Proficiency with Microsoft Office applications.
  • Knowledge of State , RBHA, and ACC requirements .
  • Ability to cultivate a culture of respect and service excellence through professionalism, skilled communication, and demonstrated commitment to integrity, trust, respect, and equity.
  • Excellent customer service and interpersonal skills.
  • Strong documentation and computer literacy.

Duties and Responsibilities:

  • Coordinates services among internal providers, primary care, specialty care, social services, and community resources.
  • Collaborate with treatment teams to review clinical needs, assess risk and implement safety plans as needed.
  • Serve as the primary point of contact between MHC healthcare and local hospitals or inpatient psychiatric units.
  • Provide timely and quality assessments, service planning, service reviews, discharge planning, and aftercare planning within a recovery model of care.
  • Ensure timely follow-up with clients’ post-discharge to support engagement, reduce hospital readmission and adhere to RBHA/ACC guidelines.
  • Serve as the daily point of contact for incoming information, referrals, and coordination needs.
  • Triage requests from internal and external sources, routing them to appropriate staff or teams.
  • Identify urgent issues requiring escalation, crisis intervention, or clinical consultation.
  • Provide direct behavioral health services to individuals, and/or family members, ensuring contact and documentation are maintained in accordance with guidelines.
  • Respond to client crises, including assessing the situation, documenting incidents, and taking appropriate action.
  • Participate in Adult Recovery Team (ART) meetings as requested or when appropriate for discharge planning.
  • Communicates all behavioral health inpatient admissions, discharges, and significant changes to the Utilization Management Team.
  • Provide clients with appropriate referrals to other community agencies as needed or requested.
  • Maintains appropriate direct service hours and ensures timely access to care per RBHA/health plans and private insurance standards.
  • Acts as a liaison between MHC Behavioral Health Services and external agencies including schools, , RBHA/ACC staff, and courts..
  • Maintain behavioral health and educational standards by attending in-services, workshops as indicated in staff development plan, and reviewing current literature.
  • Maintains confidentiality of client and family information.
  • Follow incident reporting procedures for unsafe or problematic incidents involving patients, clients, and staff.
  • Completes internal and external incident reports accurately and ensure proper follow-up, including notification of the UM/MM office.
  • Foster positive relationships with community members and other agency personnel to promote trust, productivity, and growth.
  • Demonstrate collaboration and professionalism in all responsibilities and interactions with MHC staff.
