We are hiring for an After-Hours Registered Nurse.

Responsibilities

The After-hours Registered Nurse's primary focus is to conduct comprehensive and timely delivery of hospice care for patients and their families outside regular hours, including nights and weekends.

  • Able to triage patient needs over the phone until an in-person visit by a nurse.
  • Responds appropriately and with urgency to crisis calls from patients and families after hours.
  • Makes initial nursing evaluations to determine eligibility for hospice services, including patient visits within 48 hours of referral if assigned.
  • Performs after-hours visits such as admissions, death visits, recertifications, and routine visits.
  • Identifies and reassesses patient/family physical, psychosocial, emotional, and environmental needs at least every 15 days.
  • Documents problems, goals, interventions, and responses regarding hospice care.
Education and Experience

Experience Requirements

  • Minimum of one year of clinical experience.

License Requirements

  • Current RN licensure in the state of practice.
  • Current CPR certification.
  • Valid driver’s license, vehicle insurance, access to a dependable vehicle or public transportation.
