This role has been created to support the implementation of a community care pathway for continence. In partnership with urology services Mercy university hospital (MUH), the candidate ANP will work collaboratively to develop a community continence service.
The Sláintecare Report (2017) and Sláintecare Implementation Strategy (2018) signal a new direction for the delivery of health and social care services in Ireland with the potential to create a far more sustainable, equitable, cost effective system and one that delivers better value for patients and service users. At its core, the strategy focuses on establishing programmes of work to move to a community-led model, providing local populations with access to a comprehensive range of non-acute services at every stage of their lives. This will enable our healthcare system provide care closer to home for patients and service users, being more responsive to needs and deliver better outcomes, with a strong focus on prevention and population health improvement.
Reducing our dependence on the current hospital-centric model of care and supporting capacity building in the community is key to realising the vision of Sláintecare. With our growing and ageing population, and increasing incidence of chronic disease, timely access to healthcare, aligned to general practice and delivering services at home and in the community, will not only ease pressure on our hospital system, it will better deliver what clients and service users want and need. The reform programmes aim to reduce visits to and admissions from EDs and transfer of care delays for these population cohorts. It should also lower ED waiting times more generally and the number of people on trolleys.
The overall focus for the advanced nursing service, is on providing an end-to-end pathway (within a specific scope of advanced nursing practice) for persons requiring continence care and less complex and benign urinary diseases and treatments currently been referred directing into acute services. This will in turn support the patient’s care journey and has the potential to reduce admissions to acute hospitals by providing access to diagnostics and specialist services in the community in a timely manner. For persons who require hospital admission, the emphasis is on minimising the hospital length of stay, with the provision of post-discharge follow up and support for people in the community and in their own homes, where required. A shared local governance structure across the IHAs and hospital group will support the development of a fully integrated service and end-to-end pathway.
