Overview

University Hospital Waterford is a Model 4 teaching hospital that provides general medical, surgical, paediatric and maternity care to people living in South Kilkenny, Waterford City and County. The hospital provides specialty services to the population of the southeast (c. 500,000) in areas including Cardiology, Trauma Orthopaedics, Ophthalmology, Neurology, Nephrology, Rheumatology, Urology, Vascular Surgery, ENT, and Neonatology.

Principal Duties and Responsibilities

The post holder’s practice is based on the five core concepts of Clinical Nurse Specialist role as defined by NCNM 4th edition (2008): Clinical Focus; Education and Training; Audit and Research; Consultancy. The Clinical Nurse Specialist will:

  • Provide a point of telephone contact for patients experiencing disease/treatment related symptoms and assess patients using the UKONS telephone triage system.
  • Participate in planning and carrying out unscheduled reviews of patients presenting unwell to the Oncology/Haematology Day Ward with disease/treatment related side effects.
  • Provide support to families and carers, through listening, problem solving and education.
  • Provide outreach support for patients at home.
  • Assess patients regarding the need for referral to specialist cancer services and report to the consultant/clinical leader.
  • Coordinate relevant services in preparation for treatment and attend multidisciplinary team meetings.
  • Participate in ongoing patient education related to disease management and side effects.
  • Maintain accurate clinical records in compliance with legislation and best practice.
  • Demonstrate specialist knowledge and clinical expertise in cancer patient care and ensure policies/procedures are observed.
  • Collaborate with other health care professionals and contribute to nursing guidelines and policy development for oncology patients.
Indirect Care
  • Identify and agree appropriate referral pathways for Oncology/Haematology patients with complications.
  • Participate in case reviews with MDT colleagues and use case management for patients with complex needs.
  • Lead in development of evidence-based guidelines and ensure services align with best practice and safety standards.
Education & Training
  • Assist in the development and delivery of training programmes for nursing staff and other disciplines as required.
  • Maintain up-to-date care through ongoing education opportunities.
Audit, Research & Quality
  • Ensure practice aligns with best practice and participates in audit and research related to oncology/haematology care.
  • Collect and maintain data on service impact; develop and maintain quality assurance programmes centered on patient outcomes.
  • Promote research-based practice and develop policies and standards for cancer nursing services.
  • Evaluate audit results to identify quality improvement opportunities in collaboration with nursing management and MDT colleagues.
Consultancy & Professional Practice
  • Provide clinical leadership and act as a resource for cancer services; contribute to development of clinical standards and guidelines.
  • Foster partnerships with primary care and other health professionals to ensure appropriate care for patients and families.
  • Participate in national clinical care programmes and models of care for patients and families.
Health, Safety & Quality

Role performed in line with local organisational and HSE health and safety policies; comply with Health, Safety and Welfare at Work Act (2005) and local safety statements.

Quality, Risk and Safety Responsibilities
  • Participate in quality, risk and safety initiatives and audits as required.
  • Maintain incident/near-miss reporting and adhere to relevant policies and guidelines.
  • Adhere to HIQA standards and HSE protocols; contribute to safe professional practice and hygiene standards.
Management/Administration
  • Provide an efficient, effective service; manage time and caseload to meet changing needs.
  • Maintain confidentiality and accurate records; represent the service at appropriate forums.
  • Support sustainability initiatives and comply with NCCP/HSE requirements.
