We are seeking a skilled Social Care Professional to join our team. The ideal candidate will be responsible for coordinating volunteer groups, promoting community projects, and providing support services.
Key Responsibilities :
- Coordinate the volunteer group to foster their development and promote self-organization.
- Promote community projects, such as the Digigran volunteer project, which involves students working with elderly residents.
- Support services of the Open Citizen Services Platform.
- Welcome residents, establish their connection to their new home, and monitor this process.
- Initiate and follow up on Individualized Care Plans upon resident admission.
- Handle admissions and referrals, including legal aspects, family support, and resident care.
- Address social needs of residents and obtain external aid if necessary.
- Manage administrative procedures with Public Authorities for residents with public funding.
- Follow up on residents referred to healthcare centers.
Requirements : Education :
- Diploma in Social Work is required.
- A dual qualification in Social Education is valuable.
- Training in person-centered care (ACP) and specific training in the elderly care sector are also valuable.
- Experience in residences or services aimed at the elderly is highly valued.
