Position Overview

This is an ambulatory care registered nurse position focusing on Trinity Health medical group patients, particularly those with multiple chronic diseases such as CHF and COPD. The role involves disease education, social determinants of health assessment, care coordination, and collaboration with provider offices to ensure integrated care and improved patient outcomes.

Qualifications

• Associates Degree required; BSN preferred
• Minimum of 5 years RN experience, with prior care management experience in disease management, home health, hospital, or physician office settings
• EMR experience required; project leadership experience preferred
• Certification in Case Management (CCM) preferred
• Strong communication, customer service, organizational, and behavioral skills
• Knowledge of community resources, insurance plans, and clinical resource management
• Ability to work independently and in teams, with cultural competence

Compensation

Pay Range : $69,992 - $98,072, commensurate with experience, skills, and education. Pay is based on the Fair Labor Standards Act (FLSA) guidelines and may vary by location.
