Job summary

McArthur Aged Care is seeking an experienced Clinical Care Partner to coordinate person-centred clinical care and support services for clients receiving care through the Support at Home program.

As a permanent role, you will work 3-4 days per week, collaborating with clients, families, care workers, and health professionals to help older people maintain independence and quality of life while remaining safely at home.

Qualifications

• Registered Nurse with current AHPRA registration is required.
• Experience in aged care, community health, home care, or clinical case management preferred.
• Strong assessment, care planning, and stakeholder engagement skills.

Responsibilities

• Coordinate person-centred clinical care and support services.
• Conduct clinical assessments and develop individualised care plans.
• Coordinate clinical, allied health, and community support services.
• Monitor client health, manage clinical risks, and facilitate referrals.
• Support clients and families through care decisions and transitions.
• Ensure compliance with aged care standards and governance requirements.
• Manage care delivery within funding allocations and program guidelines.

• Supporting clients and families through care decisions and service transitions
• Ensuring compliance with Aged Care Quality Standards and clinical governance requirements

About you:

• Available to work 3-4 days a week
• Registered Nurse with current AHPRA registration
• Experience in aged care, community health, home care, or clinical case management
• Strong assessment, care planning, and stakeholder engagement skills
• Sound understanding of aged care standards and person-centred care principles
• Current driver's licence and relevant clearances
