Job summary

21st Century Home Health Services (21HHS) is seeking a Part-time Registered Nurse Case Manager to serve the Marin area. You will visit patients in their homes, perform SOC admissions, and coordinate Start of Care assessments while collaborating with a multidisciplinary team.

The role requires Saturdays and Sundays on full-day shifts, with potential additional weekdays. You will have mileage reimbursement, a company-issued iPad, and a comprehensive benefits package.

Qualifications

• CA RN license and BLS/CPR certification required.
• Experience conducting SOC/OASIS assessments is highly desired.
• Experience with IV medications and high-acuity patients is a plus.
• Strong interpersonal and collaboration skills with an interdisciplinary team.

Responsibilities

• Conduct Start of Care (SOC) admissions and OASIS assessments.
• Perform follow-up visits and evaluations in designated territory.
• Make home visits and manage patient care within territory.
• Administer IV medications and monitor patient responses.
• Document observations and coordinate care with clinicians and physicians.
