Job summary

BAYADA Home Health Care is seeking a Per Diem On-Call Registered Nurse (RN) to provide evening home health care for adults and seniors in Kent County, DE. The role includes Start of Care admissions as needed and support from an experienced clinical team.

The schedule is Monday through Thursday, 5:00 PM–8:30 AM with on-call duties during those hours. The role offers an on-call stipend and visit-based pay, plus comprehensive benefits and ongoing professional support.

Qualifications

• Current RN license in the State of Delaware.
• Minimum of one year of home health nursing experience.
• Experience completing OASIS documentation and Start of Care visits.

Responsibilities

• Respond to after-hours patient needs within the assigned coverage area.
• Deliver skilled nursing care, including medication administration, wound care, and other prescribed interventions.
• Monitor patient conditions and communicate changes to the Clinical or Client Services Manager.
• Follow physician orders and ensure timely documentation.
• Coordinate care with physicians, caregivers, and the interdisciplinary team.
• Document all patient care using an EMR system.

Evening On-Call | Kent County, DE.

BAYADA at Bayhealth is seeking an experienced Per Diem On-Call Registered Nurse (RN) to provide evening home health coverage for adult and geriatric patients throughout Kent County, DE.

In this flexible on-call role, you'll provide in-home skilled nursing visits, complete Start of Care admissions as needed, and deliver exceptional patient-centered care while being supported by an experienced clinical team.

Schedule

• Monday through Thursday
• 5:00 PM – 830 AM
• On-call during scheduled hours
• Conduct in-person home visits as needed
• Receive both an on-call stipend and visit-based compensation

Responsibilities

As a Per Diem On-Call RN, you will:

• Respond to after-hours patient needs within your assigned coverage area

• Follow physician orders and ensure timely, accurate documentation
• Coordinate care with physicians, caregivers, and the interdisciplinary team
• Document all patient care using an electronic medical record (EMR) system

Qualifications

• Current Registered Nurse (RN) license in the State of Delaware.
• Minimum of one year of recent, verifiable Home Health nursing experience
• Experience completing OASIS documentation and Start of Care visits required
• Strong assessment, clinical decision-making, and communication skills
• Comfortable using electronic medical records (EMR); Homecare Homebase (HCHB) experience is a plus
• Reliable transportation and ability to travel throughout Somerset, Union, Middlesex, and Monmouth Counties

Additional Information

Base Pay, depending upon qualifications, status, and subject to negotiation:

• Weekday Stipend $100 per day
