Jobtailor is seeking a registered nurse to provide after-hours hospice care guidance by phone, triage calls, and coordinate field visits.

You will collaborate with the care team, educate families on end-of-life care, and document in the EMR while ensuring timely meds and supplies. A multistate RN license and CPR are required; 2 years hospice experience is preferred.

Qualifikationen

• Associate’s degree in nursing with active RN licensure.
• Current RN licensure with an active multistate license or ability to obtain one.
• Current CPR certification.
• Valid driver’s license and reliable transportation for home visits.
• Bachelor’s degree preferred.
• Minimum of two years hospice or palliative care experience preferred.
• Experience in telephone triage, critical care, or emergency care preferred.

Aufgaben

• Serve as primary contact for hospice patients and families during after-hours, weekends, and holidays.
• Triage incoming calls, assess patient needs, and provide clinical guidance by phone.
• Coordinate after-hours visits by hospice field nurses or other team members.

• Provide symptom-management instructions for pain, breathing difficulties, nausea, anxiety, and other concerns.
• Offer emotional support to families and caregivers.

• Respond to emergencies and dispatch hospice field staff when necessary.
• Collaborate with physicians, social workers, chaplains, nurses, and other team members.
• Update plans of care reflecting patient status and maintain interdisciplinary communication.
• Ensure timely ordering and availability of medications and medical supplies.

Job Responsibilities

• Serve as the primary point of contact for hospice patients and families during after-hours, weekend, and holiday coverage


• Triage incoming calls, assess patient needs, and provide clinical guidance and interventions by phone


• Resolve or route non-clinical requests involving durable medical equipment, pharmacy, scheduling, and general questions


• Collaborate with physicians, social workers, chaplains, nurses, and other hospice team members


• Make clinical decisions based on patient status and plans of care


• Provide symptom-management instructions for pain, breathing difficulty, nausea, anxiety, and other concerns


• Offer emotional support and guidance to families and caregivers


• Educate patients and families on end-of-life care, medication administration, and comfort measures


• Respond to emergencies, guide caregivers through symptom-control techniques, and dispatch hospice field nurses when necessary


• Coordinate after-hours visits by hospice field nurses or other team members


• Ensure timely ordering and availability of medications and medical supplies


• Document triage interactions and nursing assessments in the electronic medical record (EMR)


• Complete workflow


• Provide detailed reports to registered nurse teams, hospice physicians, and case managers


• Update plans of care to reflect changes in patient status and maintain interdisciplinary communication

Requirements

• Associate’s degree in nursing


• Current RN licensure with an active multistate (compact) license required, or willingness to obtain one


• Current CPR certification


• Valid driver’s license and reliable transportation for potential home visits


• Bachelor’s degree preferred


• Minimum of two (2) years of hospice or palliative care experience preferred


• Experience in telephone triage, critical care, or emergency care preferred
