NHS Lanarkshire is seeking a split Band 7 post for a rotational clinical pharmacist (0.5 WTE) and 0.5 WTE supporting HEPMA, based at University Hospital Monklands. You will help develop, deliver and maintain the clinical aspects of HEPMA and contribute to ongoing research and evaluation.

The role involves training, system maintenance, audits, and participation in on-call rota, with emphasis on clinical care, education, and multidisciplinary collaboration within the HEPMA team across Lanarkshire

Formación

• Masters of Pharmacy Degree
• Registered member of General Pharmaceutical Council
• Independent prescribing qualification or must work towards
• Evidence of postgraduate training and professional development in clinical pharmacy practice e.g. Post Registration Foundation Framework/ 1st year clinical diploma

Responsabilidades

• To deliver patient-centred specialist pharmaceutical care to patients in specified clinical area(s).
• To communicate effectively, placing the person at the centre of any interaction.
• To deliver patient education to ensure the safe and effective use of medications and devices.
• To collaborate with the wider pharmacy and multidisciplinary team; actively participating in consultant ward rounds and multidisciplinary team meetings.
• To deliver specialist clinical care to patients by assessment of their pharmaceutical care needs and developing and actioning a care plan.
• To carry out consultations with patients in pharmacist-led clinics and in ward, prescribing where appropriate.
