Job summary

UK St. Claire is seeking an experienced registered nurse to provide compassionate, patient-centered hospice care. You will assess and manage pain, coordinate with physicians and interdisciplinary teams, and support families throughout the hospice journey.

Responsibilities include documenting patient needs, educating families, and ensuring compliance with regulations. A valid RN license is required, and the role includes an on-call weekend rotation with a Monday–Friday schedule.

Qualifications

• Associate’s degree in nursing.
• Bachelor’s degree in nursing preferred.
• 0–2 years of nursing experience.
• Active RN license.
• Equivalent combination of education and experience may be considered.

Responsibilities

• Provides direct nursing care to hospice patients in home or facility settings according to care plans.
• Assesses, monitors and documents patient symptoms, comfort levels and response to interventions.
• Administers medications and treatments to manage pain and other end-of-life symptoms.
• Collaborates with physicians and interdisciplinary team members to develop and update individualized care plans.
• Educates patients, families and caregivers on symptom management and end-of-life care practices.
• Supports emotional, psychosocial and spiritual needs of patients and families.
• Ensures adherence to hospice regulations, clinical policies and infection control standards.
• Coordinates care transitions and communicates effectively with care teams and external partners.
• Participates in case conferences, quality improvement activities and professional development.
• Performs other duties as assigned.
