Well Care Health is seeking a Hospice Nurse Practitioner to provide advanced clinical assessment and symptom management for hospice patients in collaboration with the IDG and Hospice Medical Director. You will participate in eligibility determinations, education, and after-hours coverage as needed.

As a clinical resource, you will mentor staff, ensure timely documentation, and support transitions across hospice care levels while adhering to regulations and organizational standards.

Formación

• Graduate of NLN accredited school of nursing and current license to practice professional nursing as a Registered nurse in the state; bachelor’s degree preferred.

Responsabilidades

• Provide advanced assessment, evaluation, and management of terminal illness symptoms including pain, dyspnea, agitation, nausea, anxiety, wounds.
• Conduct Face-to-Face hospice eligibility and recertification encounters per Medicare and policies.
• Collaborate with Hospice Medical Director and physicians on eligibility, symptom management, and medication adjustments.
• Develop, review, and revise individualized plans of care with the IDG, patient, and family.
• Participate in IDG meetings, offering clinical recommendations for patient care needs and eligibility.
