Job summary

Well Care Health in Switzerland is seeking a dedicated Hospice Registered Nurse Case Manager to provide compassionate end-of-life care, manage a patient caseload, and coordinate interdisciplinary services with the hospice team.

As a key member of our nursing staff, you will assess needs, develop individualized plans of care, educate families about hospice, and support patients through life’s final stages with empathy and clinical excellence.

Qualifications

• Graduate of NLN accredited school of nursing and current Registered Nurse license.
• Bachelor’s degree in nursing preferred.
• Hospice experience preferred.
• Strong interpersonal and teamwork abilities.

Responsibilities

• Complete initial, comprehensive, ongoing patient and family assessments and provide nursing services per care plan.
• Develop individualized plans of care with hospice team and family, incorporating palliative interventions.
• Communicate with attending physicians regarding patient needs and changes in condition.
• Collaborate with the interdisciplinary team to provide holistic care and bereavement support.
• Document care activities and maintain accurate EMR records; supervise assigned staff as needed.
