Our compassionate and experienced staff will welcome you to join our team and work with them to provide top-of-the-line care to our residents. Requirements for MDS Coordinator:

• Must possess valid unencumbered LPN or RN State License and current CPR certification
• Nursing degree from an accredited college or university
• At least two (2) years of MDS experience preferred

Job Duties for MDS Coordinator:

• Accurate completion of RAI for all Medicare patients in the facility
• Provides completion of the nursing section of all Medicare MDSs and RAPs
• Complete OBRA MDSs as PPS caseload allows
