A non-profit advocacy organization in Manitoba seeks a Case Manager to provide support to Indigenous youth transitioning into adulthood. The role requires strong communication skills, experience in social services, and the ability to develop personal support plans. Applicants should have a Bachelor's degree in social work or related fields and relevant work experience. This position offers a full-time opportunity to make a meaningful impact in the community.
Qualifications
- Certificates or experience within communications, client assessments, life coaching and mental health.
- Previous experience in case management or social services, particularly with transitioning youth.
- At least 2 years of front-line service delivery experience.
Responsabilités
- Provide direct support to clients transitioning into adulthood.
- Conduct assessments and develop personalized support plans.
- Maintain documentation of client intake and recommendations.
