A healthcare organization is seeking a Social Worker in Northwestern Ontario to provide comprehensive assessments and interventions to clients and families. The ideal candidate will have a Baccalaureate degree in Social Work and experience in healthcare settings, with a commitment to diversity and community support. The Social Worker plays a key role in supporting clients, their families, Substitute Decision Makers (SDMs), and other members of their support network. In this role, you will provide comprehensive assessments and interventions to address psychosocial needs, while contributing a social work perspective as part of an interprofessional team. You will also act as a liaison and consultant to internal and external programs within your scope of practice and actively participate in quality improvement initiatives.
The focus of Social Work practice in this program is to restore, maintain, and enhance quality of life by building on client strengths, supporting coping strategies, helping individuals adapt patterns of interaction and daily living, connecting people to necessary resources, addressing environmental stressors, and providing education that promotes wellness and wellbeing.
