Job summary

Shared Health in Manitoba seeks an experienced Utilization Facilitator to coordinate patient movement across Adult Medicine and Family Medicine, including admission, transfer and discharge. The role requires RN with Bachelor of Nursing and active CRNM registration, plus CPR compliance as per policy.

Strong communication skills and ability to collaborate across interdisciplinary teams are essential; Indigenous language knowledge is preferred.

Job description

Shared Health leads the planning and coordinates the integration of patient-centered clinical and preventive health services across Manitoba. The organization also delivers some province-wide health services and supports centralized administrative and business functions for Manitoba health organizations.

Position Overview

Under the general direction of the Utilization Manager and Facility Program Director(s) - Medicine/Family Medicine and in close collaboration with the Facility Vice President responsible for Utilization, Physician Bed Manager(s), and Clinical Manager(s), the Utilization Facilitator is responsible for:

  • Facilitating and coordinating the movement of patients through the admission, transfer, and discharge processes primarily within the Adult Medicine/Family Medicine Program, ER Program, and other clinical programs as required.
  • Liasing between internal and external programs and agencies on matters related to patient movement.
  • Participating in the development of guidelines and educational strategies related to patient movement.
  • Participating in decision-making that results in an effective admission process and the optimal utilization of Program and facility resources.
Experience
  • Minimum 4 years recent nursing experience.
  • Recent acute clinical experience preferred.
Education (Degree/Diploma/Certificate)
  • Graduate from a recognized program in Nursing.
  • Bachelor of Nursing degree required.
  • Educational preparation in utilization an asset.
Certification/Licensure/Registration
  • Responsible for maintaining and providing proof of active registration with the College of Registered Nurses of Manitoba (CRNM).
  • Active participation in professional associations e.g. Association of Regulated Nurses of Manitoba (ARNM) preferred.
  • Cardiopulmonary Resuscitation (CPR) training requirements for this position shall be in accordance with the Employer policy.
Qualifications and Skills
  • Scope of practice as documented in the Regulated Health Professions Act (RHPA).
  • Demonstrated ability in utilizing computerized applications, such as Microsoft Word and Excel.
  • Demonstrated ability to function in a pivotal role, as facilitator in the patient admission/discharge/transfer process between patient, family, physician, Clinical Manager, members of the Interdisciplinary Care Team, other programs, community resources, etc.
  • Demonstrated ability to initiate and maintain effective resource contacts.
  • Demonstrated ability to work with all levels of staff (direct care, interdisciplinary team members and management)
  • Demonstrated ability to analyze and problem-solve
  • Excellent interpersonal and communication skills with co-workers, management, patients and families.
  • Preference will be given to those applicants competent in an Indigenous language and/or knowledge in Indigenous customs, traditions and values.

This position requires a current satisfactory Criminal Records Check (including Vulnerable Sector Search), Child Abuse Registry Check and Adult Abuse Registry Check as conditions of employment. The successful candidate will be responsible for any service charges incurred. A security check is considered current if it was obtained no more than six (6) months prior to the start of employment.
