Job summary

Shared Health in Winnipeg seeks a registered nurse to coordinate patient movement across Adult Medicine/Family Medicine and ER programs, ensuring smooth admission, transfer and discharge. The role is in person at Health Sciences Centre and 0.70 FTE with day, evening, and weekend shifts.

You will liaise with internal and external teams, develop guidelines for patient movement, and help maximize program and facility resource use.

Qualifications

  • Graduate from a recognized program in Nursing.
  • Bachelor of Nursing degree is required.
  • CRNM registration and CPR training per employer policy.

Responsibilities

  • Facilitate and coordinate patient movement through admission, transfer, and discharge.
  • Liaise between internal/external programs and agencies on patient movement.

Shared Health leads the planning and coordinates the integration of patient-centered clinical and preventive health services across Manitoba. The organization also delivers some province-wide health services and supports centralized administrative and business functions for Manitoba health organizations.

Position Overview
  • Facilitating and coordinating the movement of patients through the admission, transfer, and discharge processes primarily within the Adult Medicine/Family Medicine Program, ER Program, and other clinical programs as required.
  • Liasing between internal and external programs and agencies on matters related to patient movement.
  • Participating in the development of guidelines and educational strategies related to patient movement.
  • Participating in decision-making that results in an effective admission process and the optimal utilization of Program and facility resources.
Experience
  • Minimum 4 years recent nursing experience.
  • Recent acute clinical experience preferred.
Education (Degree/Diploma/Certificate)
  • Graduate from a recognized program in Nursing.
  • Bachelor of Nursing degree required.
  • Educational preparation in utilization an asset.
Certification/Licensure/Registration
  • Responsible for maintaining and providing proof of active registration with the College of Registered Nurses of Manitoba (CRNM).
  • Active participation in professional associations e.g. Association of Regulated Nurses of Manitoba (ARNM) preferred.
  • Cardiopulmonary Resuscitation (CPR) training requirements for this position shall be in accordance with the Employer policy.
Qualifications And Skills
  • Scope of practice as documented in the Regulated Health Professions Act (RHPA).
  • Demonstrated ability in utilizing computerized applications, such as Microsoft Word and Excel.
  • Demonstrated ability to function in a pivotal role, as facilitator in the patient admission/discharge/transfer process between patient, family, physician, Clinical Manager, members of the Interdisciplinary Care Team, other programs, community resources, etc.
  • Demonstrated ability to initiate and maintain effective resource contacts.
  • Demonstrated ability to work with all levels of staff (direct care, interdisciplinary team members and management)
  • Demonstrated ability to analyze and problem-solve
  • Excellent interpersonal and communication skills with co-workers, management, patients and families.
  • Preference will be given to those applicants competent in an Indigenous language and/or knowledge in Indigenous customs, traditions and values.
