Job summary

CareKW is seeking experienced Unregulated Care Providers (UCP) to deliver client-centered care in facility and community settings across Ontario. The role requires collaboration with a nursing team and flexible shifts to ensure coverage in the community.

Ideal candidates have prior care experience, CPR/First Aid, immunizations, and a willingness to commute between Brantford, St. Catharines, and Hamilton, with occasional trips to Kitchener as needed.

Qualifications

• Previous experience in providing person care.
• Preferred PSW/HCA/DSW certification.
• Hold a valid G drivers license and willing to commute between Brantford, St Catharines and Hamilton.
• Current CPR/First Aid Certification.
• Clean Vulnerable Sector Screening (within 3 months of hire).
• Up-to-date Immunization Records (MMR, TDAP, Varicella, Hep B).

Responsibilities

• Complete regular caregiver skill evaluations in collaboration with nursing team.
• Address client or caregiver queries related to care delivery (in person or by phone).
• Perform client intake assessments and update assessments as needed; communicate focus of intervention.
• Create and maintain client care plans; inform community care coordinator of changes.
• Be flexible to cover open shifts and adjust work schedule for coverage in the community.
• Work with clients to establish goals to increase function at home.
• Work as part of a multidisciplinary team.
• Assist with policy creation as needed.
• Be responsive to emails and calls; provide client-centered, family-centered care.
• Must be available for rotating shifts.

Unregulated Care Provider (UCP):

CareKW is looking for experienced, skilled and reliable individuals to join our team as a UCP to provide support in Facility and Community settings.

CareKW is focused on creating a team of qualified individuals to enhance and advance the health of the community. We are open to UCP's with a variety of backgrounds and training expertise.

Shift Details:

Employment Type: Full time
Pay Range: $20 - $23/ hour
Shift timing: 8 hour - 12 hour rotational shifts between 6AM and 11PM
Schedule:10 shifts biweekly, every other weekend required

Responsibilities:

• Complete regular caregiver skill evaluations, address concerns as appropriate in collaboration with nursing team.
• Address client or caregiver queries or concerns related to the provision of care (in person or by telephone)
• Perform client intake assessments prior to the start of services and complete client update assessments as needed throughout the duration of service; communicate focus of intervention to client and family
• Create and maintain client care plans based on client’s identified care needs; communicate changes in client’s health status or discrepancies in assigned care tasks to community care coordinator
• Be flexible in covering open shifts and display willingness to adjust work schedule to ensure coverage in the community
• Work with clients to establish meaningful goals to increase their function at home
• Work as a dynamic part of a multidisciplinary team
• Assist with policy creation as needed
• Be responsive to emails and calls as needed
• Provide client-centered, family-centered care
• Must be available for rotating shifts.

Requirements:

• Previous experience in providing person care
• Preferred post-secondary education in related care fields of PSW/HCA/ DSW certification
• Hold a valid G drivers license and willing to commute between Brantford, St Catharines and Hamilton
• Willing to commute to Kitchener once per week
• Valid work/study permit (if applicable)
• Current CPR/First Aid Certification
• Clean Vulnerable Sector Screening (within 3 months of hire)
• Up-to-date Immunization Records (MMR, TDAP, Varicella, Hep B)
• Gentle Persuasive Approach (GPA) considered an asset
• Non-Violence Crisis Intervention (NVCI) considered an asset
• Documentation skills
