Sustained growth has resulted in the need to expand our team of Therapists within the Addiction Medicine Program. As a Therapist, Mental Health & Addiction, you will be an integral part of a collaborative, interdisciplinary team focused on delivering seamless services and programs to our clients and customers. This role involves treatment planning, clinical case coordination, and direct service through individual and group work. It is ideal for Mental Health Therapists, Psychotherapists, or Social Workers with psychotherapy experience, who thrive in a dynamic, fast-paced environment helping individuals with complex mental health and/or addictions in a concurrent disorders program.

Hours of work: Master schedule with evenings and weekend rotation as per schedule.

What you’ll be doing:
  • Coordinate, initiate, and may conduct assessments to guide treatment planning.
  • Provide case management services including service planning, coordination, navigation, and evaluation of progress.
  • Deliver individual and group psychotherapy utilizing evidence-informed practices such as Motivational Interviewing, Stage-Matched Intervention, Trauma-Informed approaches, Brief Solution-focused Therapy, CBT, DBT, CPT, and other evidence-based practices where applicable.
  • Offer support to family members through individual and group sessions, and lead counseling and education groups.
  • Collaborate closely with physicians and the clinical team to monitor patient progress, review treatment plans, and liaise with referral sources.
  • Develop and implement discharge planning and facilitate referrals to community resources.
  • Identify discharge needs based on comprehensive assessments and care plans.
  • Provide service and team leadership by establishing treatment goals and working with patients and team members to develop action steps.
  • Lead team monitoring and evaluation of treatment progress.
  • Act as a communication lead among patients, the team, and programs, ensuring critical information is effectively communicated.
  • Complete documentation and record-keeping following Homewood Health Centre and College policies.
What we’re looking for:
  • Bachelor’s or Master’s degree in a recognized Social Science program with addiction-specific education.
  • Membership in good standing with the relevant regulatory college in Ontario or pursuing relevant credentials such as OCSWSSW, COTO, CNO, or registration as a Registered Psychotherapist (RP) with the College of Registered Psychotherapists.
  • Competence in delivering CBT, DBT, and other evidence-based approaches for complex mental health and substance use disorders.
  • Experience in assessment and treatment using evidence-based practices.
  • Recognition of the value of measurement-based care to guide clinical decisions; experience is an asset.
  • Exceptional communication and interpersonal skills.
  • Ability to facilitate and lead an interdisciplinary team, coordinating team resources.
  • Skills in individual and group counseling, including psycho-education for large groups.
  • Ability to deliver inclusive and culturally responsive approaches.
  • Knowledge of addictions and concurrent issues across a continuum of care.
  • Valid Ontario driver’s license and access to a vehicle if required.
  • Knowledge of the Mental Health Act, PHIPA, Ontarians with Disabilities Act 2005, and Health Care Consent Act.
