Reporting to the Territorial Director Mental Health and Addictions, the Territorial Client Care Coordinator (Coordinator) is the member of Territorial Mental Health and Addictions team who is specifically tasked with linking and navigating complex systems for both domestic and out of territorial resources. This position is responsible for all aspects of case management for Nunavut based Mental Health and Addiction Clients receiving treatment throughout Nunavut and Out-of-Territory (OTT). The primary goal of this position is to support client and family well-being while they are progressing through multiple systems of care.
The Territorial Client Care Coordinator will be responsible for providing ongoing support, assessment and coordination between the clinical teams, multiple government of Nunavut departments, various local and OTT treatment centers and facilities through planning, implementing, communicating, monitoring, and evaluating actions taken to meet the client and family community needs. The Coordinator is also responsibility for ensuring the services and supports are identified in the client's plan of care and are the right services. This is done in conjunction with the assigned mental health clinician in-Territory and the Pre-Placement Review Committee (PPRC). The Coordinator provides support, guidance, and supervision, for alternate Treatment Centres and Independent Living Arrangements as well as ensuring all contracts are being maintained.
The knowledge, skills, and abilities required for this job are usually obtained through a recognized undergraduate degree in a health-related field, including social work along with two (2) years of mental health, or addictions experience within a clinical environment. This is a Highly Sensitive Position and a satisfactory Criminal Record Check, along with a clear Vulnerable Sector Check is required.
The Official Languages of Nunavut are Inuktut, English and French. Applicants may submit their resume in any of the official languages of Nunavut. Fluency in more than one of Nunavut’s official languages would be considered an asset. Knowledge of Inuit communities, culture, land,Inuit Qaujimajatuqangit, Inuktut and experience working in a northern cross-cultural environment are also considered assets.
