We’re looking for a Hospice Palliative Care Coordinator who shares our passion for care to join our team.
As a member of The Dorothy Ley Hospice (DLH) Community Team you will support individuals and those close to them navigate the palliative care process and develop personalized care plans based on individual needs, preferences, hopes and goals. Individual-centered care ensures respect, dignity and choice. As our Hospice Palliative Care Coordinator you will work closely with colleagues in Home and Community Care Support Services, the DLH Community Physician Team, and the DLH Community Team to provide holistic end of life care in the home.
Environment: In the community (home setting of persons served), may include other health care settings; and at The Dorothy Ley Hospice.
Position: Full-time position (37.5hrs/week), 1-year term with possibility for extension.

Established in 1990, Dorothy Ley Hospice was created for the community, by the community. What started as an ambitious desire of a small group of community members to provide compassionate end of life care is now an award winning, not-for-profit, and acclaimed outreach and on-site hospice care facility committed to providing compassionate, palliative care to individuals facing life-limiting illness and delivering bereavement services to people learning to live with grief and loss. With funding from community grants, government bodies, private and corporate donors alongside a dedicated community of highly regarded palliative care specialists and healthcare professionals, staff and volunteers, our 12-bed residence and community care teams serve over 2,000 community members each and every year. www.dlhospice.org
