CBI Home Health in Oshawa region is seeking a Speech Language Pathologist to provide home-based assessment and treatment, including swallowing and communication interventions, within an Integrated Care Program.

You will work with a collaborative team to deliver personalized, confidence-building care in the community, using digital platforms for documentation and care delivery. A licensed SLP in good standing is required.

Qualifications

• Registered with the College of Audiologists and Speech Language Pathologists of Ontario.
• Valid driver’s license and vehicle insurance.
• Ability to deliver care in person and via virtual methods.
• Commitment to person- and family-centered care.

Responsabilités

• Assess and implement SLP interventions in a home-based setting (e.g., swallowing assessments, dysphagia management, communication).
• Collaborate with an interdisciplinary team to plan and deliver care.
• Support holistic and restorative care approaches (stroke pathway, cognitive communication supports).
• Utilize digital health platforms to support care delivery, documentation, and intervention.

Salary Range: The salary range for this position is $68.24 – $70.65 per visit

What you will do

• Assessment and implementation of SLP interventions (i.e. swallowing assessments, dysphagia management, communication, etc.) in a home-based setting
• Use of a collaborative and interdisciplinary team-based approach to care and services
• Support a holistic and restorative approach to care (i.e. enhanced stroke pathway; cognitive communication supports, etc. )
• Leverage our digital health platforms to support care delivery, collaboration, documentation, and intervention

What you will need to be successful

• You are registered as a member in good standing with the College of Audiologists and Speech Language Pathologists of Ontario
• Valid driver's license and vehicle insurance
• Your focus is to embrace person and family-centered care
• You are comfortable delivering care both in person and using virtual methods/technology
• You are passionate, empathetic, use good judgement and apply critical thinking when making client decisions
• You are known for your excellent communication skills and ability to work in collaboration within an interdisciplinary team as well as work independently
• You are flexible and have demonstrated skills in critical thinking that you can apply to a variety of situations.
• You value the role a therapist plays in keeping clients at home, where they want to be
