Michael Garron Hospital is looking for a Social Worker to provide assessment, consultation, treatment, and education to patients, families, and the inter-professional team across multiple program areas including critical care, respirology, medicine, stroke, surgery, family birthing centre, and ED. Working as a member of the inter-professional team, the Social Worker will be responsible for providing assessment, consultation, treatment and education to the patient, family members and the interprofessional team within a variety of program areas including but not limited to critical care, respirology, medicine, stroke, surgery, family birthing centre & the emergency department. The successful candidate will participate in program development and evaluation as well as actively participating in regular team meetings, liaising with community-based providers, advocating on behalf of clients and their families and participation in inter-professional patient and family conferences as needed. The primary area of coverage for this role will be within theFamily Birthing Centre/Paediatrics/NICU & Emergency Department. Previous work history of dealing with one or more of the following client types/issues: elderly, housing, domestic violence, paediatric/maternal newborn care, substance abuse, and bereavementis preferred.
