This role provides professional social work services in clients’ homes, continuing care facilities, and hospital settings. As part of a multidisciplinary team, you will: Conduct psychosocial assessments, identify unmet needs and develop care plans, provide social navigation and resource-based interventions, collaborate with clients, families, and decision-makers to achieve client goals. Key Responsibilities: Crisis intervention, education, advocacy, and resource navigation financial assessment for continuing care placement, risk screening, counselling, transition and discharge planning, supporting care partners in navigating health care resources, relevant education and cognitive screening.

Required Qualifications:

Completion of bachelor's degree in Social Work.Active or eligible for registration with the Alberta College of Social Workers (ACSW).

Preferred Qualifications:

Experience in Continuing Care settings. Familiarity with Connect Care. Palliative Care experience. Knowledge of community resources, referral pathways, and interagency collaboration.
