Job summary

Assisted Living Alberta at the Glenrose is seeking a Social Worker II to support patients in an inpatient rehabilitation setting and contribute to discharge planning within a multidisciplinary team. You will perform psychosocial assessments, risk screening, counseling, and resource coordination with families, caregivers and community partners.

Qualifications include a bachelor's degree in Social Work and eligibility for ACSW registration; the role is 0.80 FTE with day shifts at Glenrose Hospital

Assisted Living Alberta at the Glenrose is looking for a Social Worker to support patients in an inpatient rehabilitation setting and be an integral part of department changes. This position requires a high degree of critical thinking, strong knowledge of Social Work scope and role, and a passion for practice excellence. You will be required to prioritize workload, formulate assessments and create intervention plans in a short timeframe. These plans include psychosocial and financial interventions, risk management and resourcing to support ongoing treatment and/or discharge planning. The Social Worker will demonstrate exceptional communication skills and ability to work collaboratively with caregivers, families, and community organizations. This position works with multidisciplinary teams and requires competence in approaching questions about decision-making and capacity. This position is highly involved with processes, services and resources related to alternate levels of care.

As a Social Worker II, you will be a member of an inter-disciplinary team and provides a range of social work services to patients and their families. Your services may include: psychosocial assessment, resource-based interventions, individual, group and family treatment, risk screening, counselling, case management and coordination, transition or discharge planning, and prevention, promotion and public awareness activities.

Required Qualifications:
