Responsibilities

• Provide basic social work and clinical counseling services to patients and families, exploring emotional responses, and facilitating change.
• Conduct social work assessments using various tools, in line with professional standards.
• Offer consultation to patients, families, and healthcare professionals on issues like high risk factors, recovery, trauma, and community support.
• Maintain patient records, document assessments, and prepare reports.
• Lead discharge planning, coordinate care plans, and collaborate with community services.
• Communicate with community agencies and facilitate referrals.
• Serve as a resource for patients and families, providing information on community resources.
• Participate in program evaluation, research, and professional development activities.
• Perform other duties as assigned.

Qualifications

Education and Experience: Bachelor's degree in Social Work, one year recent clinical experience, and registration with BC College of Social Workers.

Skills and Abilities: Knowledge of mental health issues, counseling skills, team leadership, community resource knowledge, effective communication, organizational skills, and physical ability to perform duties.
